Thomas Jay Perkins, 62, died November 22, 2016. Thomas was born in Akron and lived all of his life in Cuyahoga Falls. He was a member of the International Society of Phlebotomists.
He was preceded in death by his parents Lenox and Kathryn Perkins. Thomas is survived by his brothers Lenox III (Judy), John, and Keith (Debra); nieces Denise Williams, Heather Perkins, and Elizabeth Perkins.
Memorial services will be conducted 1pm, Wednesday, November 30, 2016 at Redeemer Lutheran Church, 2141 5th Street, Cuyahoga Falls, Ohio 44221. Memorials may be made to the American Heart Assoc., 3505 Embassy Pkwy. Suite 100, Akron, Ohio 44333 or to St. Jude Children Research Hospital, 262 Danny Thomas Place, Memphis, Tn. 38105. Services entrusted to Clifford-Shoemaker Funeral Home and Crematory, 330-928-2147.
